4.FUMINGATION BY FORMALDEHYDE

for each 1000 cubic fit of the volume of the operation theatre 500ml of Formaldehyde that is 40% solution added in 1000ml of water in a electric boiler ,After 45 minutes switch off the boiler without entering into the room 

5.FUMINGATION BY POTASSIUM PERMANGANET 

*For every 1000 cubic feet add 450 gm of KMNO4 to 500 ml of Formaldehyde (40% of solution),

*After Fumingation seal the room for 48hours

*To neutralize Formaline vapour ,place the amonia solution in the centre of the room and leave it for 3hours

1.ROOF- 2%bacilloid ,once in 3 months

2.Walls-2%bacilloid ,Daily twice

3. Floor-2%bacilloid,Daily twice

4. Refrigerator-Defrost and clean with soap and water ,weekly once

5.Sink-soap and water or Sodium hypochlorite -Daily once 

6.OT Furniture- Alchohol based spray, daily once 

7.AHU And pre filter -Water,once in 3 months

*patient has confirmed -identity,site, procedure,consent

*Anesthesia safety check completed

*Difficult airway/aspiration risk ?(No/Yes)

*Risk of >500ml blood loss (7ml/kg in children)-(No/Yes)


TIME OUT- Before skin incision 

*Confirm all team members have introduced themselves by name and role 

*Surgeon , anesthesia profession and nurse verbally confirm -patient,site, procedure

* All Anticipated critical events like surgeon, anesthesia, Nursing team reviews

SIGN OUT-Before patients leaves operationg room

Nurse verbally confirm within the team 

*The name of the procedures recorded

*That instrument,sponge,needle counts are correct

*is the specimen is labelled 

*Whether there are any equipment problem to be addressed

*Surgeon, anesthesia professional and nurses reviews the key concern
